Admission to MRSI

Application for Services

Why is it important to list the applicant's assets?

The various funding sources have very strict guidelines regarding the financial eligibility criteria that they follow. Section 5 (page 6) of the MRSI application is very important for this reason. There are options that can be looked at to ensure that once an applicant is eligible for funding, they can remain eligible. Please be sure to be accurate in reporting of assets so that the funding eligibility is not compromised in the future.

Why do I need to be thorough and detailed in completing the Application for Services?
All of the information that is requested in the application is very important. When there are blanks or inaccurate information, it can slow down the entire process. If you have any questions, please contact an MRSI Case Manager for assistance.

Why does an applicant need a current Psychological Evaluation?
The criteria for meeting eligibility for services includes the diagnosis that can only be determined through a licensed psychologist. There are also guidelines that the psychologist will need to include in the evaluation. An MRSI Case Manager can provide the psychologist with these guidelines. The family and/or applicant will not be responsible for paying for this evaluation. An MRSI Case Manager will ensure that funding is obtained to pay for the evaluation.

What is an ICAP?
ICAP stand for "Inventory for Client and Agency Planning". This booklet is completed on every applicant so that functional limitations, adaptive behavior (motor skills, social and communication skills, personal living skills, and community living skills), and problem behavior can be identified. This is also a very important tool used in determining eligibility for funding. A "team" of people will be gathered who know the applicant very well. The Area Resource Specialist will lead the meeting and make sure that all team members are in agreement with the responses given. This assessment will also help with the development of the IPC if the applicant is determined eligible and will receive services.

Who do I contact regarding an application?
The person at MRSI to contact regarding an application for services is David Pinter. David is the Case Management Director for MRSI and the Intake Committee Chairperson. All applications go to David for review and presentation to the Intake Committee.

What is Targeted Case Management?
