Community Living Support
Services
MRSI Community Living Programs
identify alternative living environments available in the
community, and those alternatives are communicated to the person
served. The Community Living Staff assists the persons served to
reside in the most independent environment or least restrictive
environment available in which their needs can be met. Each
person's living environment is evaluated for appropriateness at
least semiannually. Each person served has individually tailored
support services which reflect their needs and desires. Each
person served has assistive technology and related equipment as
appropriate to their needs and preferences within the concept of
reasonable accommodation.
Program Plans An Individual Plan of Care
will be developed with and for each person served. The plan of
care will include, among other things:
- The strengths, abilities, needs and preferences of the
person served.
- How friendships and relationships will be continued and
developed.
- Short and long term goals and objectives that are
time-phased, measurable, functional, and consistent with the
goals of integration, productivity, and independence.
- The methods to be used to achieve the goals and
objectives.
- The methods that will be used to coordinate and integrate
the Community Living Program services with the family and with
other available services.
Supervised Living Support Services Program Mountain
Regional Services, Inc. Supervised Living Program provides
individualized care, supervision, support and training to
maintain and promote self-sufficiency of persons with
disabilities. Such services are provided in homes in which
personnel are present nightly and usually at other times when
the persons served are at home. Some of the training provided
by the MRSI Supervised Living Staff include:
- Basic Self-Care Skills such as grooming, personal hygiene
and proper clothing awareness
- Communication skills
- Contingency planning, problem solving, and decision making
- Financial Management
- Health Maintenance
- Housekeeping and home maintenance skills
- Interpersonal relationships
- Menu planning and meal preparation
- Mobility and community transportation skills
- Recreational and leisure time activities
- Self-advocacy and assertiveness training
- Use of the telephone
- Utilization of community services and resources
