Community Living Support Services
MRSI Community Living Programs identify alternative living environments available in the community, and those alternatives are communicated to the person served. The Community Living Staff assists the persons served to reside in the most independent environment or least restrictive environment available in which their needs can be met. Each person's living environment is evaluated for appropriateness at least semiannually. Each person served has individually tailored support services which reflect their needs and desires. Each person served has assistive technology and related equipment as appropriate to their needs and preferences within the concept of reasonable accommodation.
Program Plans
An
Individual Plan of Care will be developed with and for each person
served. The plan of care will include, among other
things:
- The strengths, abilities, needs and preferences
of the person served.
- How friendships and relationships will be
continued and developed.
- Short and long term goals and objectives that
are time-phased, measurable, functional, and consistent with the goals
of integration, productivity, and independence.
- The methods to be used to achieve the goals and
objectives.
- The methods that will be used to coordinate and integrate the Community Living Program services with the family and with other available services.
